Responsibilities:

• Deliver safe, high-quality nursing care to patients in their homes, in accordance with the physician’s plan of care and established clinical guidelines.
• Monitor patient conditions, administer medications and treatments, and report changes in health status promptly to the RN or physician.
• Collaborate with the interdisciplinary care team, including RNs, therapists, case managers, and physicians to support individualized care planning and goal setting.
• Educate patients and family members on medications, treatments, and home care procedures to support independence and long-term health outcomes.
• Accurately document all care provided, patient responses, and communication with the care team using approved electronic charting systems.
• Participate in routine case reviews, care conferences, and quality improvement initiatives as needed.
• Maintain compliance with agency policies, state and federal regulations, and infection control standards.
• Perform additional duties as assigned by the supervising RN or clinical manager.

Requirements:

• Current LVN License in the state of Texas
• Proof of current CPR Certification.
• Minimum of one year of experience as a LVN.
• Previous experience in Geriatric Home Health.
• Kinnser experience is a plus.
• Must have and maintain a valid Texas driver’s license and proof of current auto liability insurance.
